My Mother isa kind and gentle woman She is always very gentle She takes good care of her children and keeps them all at school I have one brother and two sistets So she gets four children in all She gives us every comfort We all love her and she loves us also

故事发生美国60年代，1964年。讲述马萨诸塞州的一间疗养院「荆棘崖 Briarcliff」里所发生的光怪陆离的事件[2]。荆棘崖里只有三种人－医疗人员（医生和护士）、神职人员（神父与修女）以及精神病患（罪犯）。偌大的疗养院运作由严峻冷酷的修女茱德（洁西卡·兰芝）和她的门生玛莉·尤妮丝（莉莉·拉贝），以及疗养院创始人，亦为主教的提摩西·霍华德（乔瑟夫·范恩斯）共同负责。而在院内治愈患者的医生包括－精神科医生．奥利佛·崔德森（沙查·昆图）和残暴的科学家兼主治医生亚瑟·亚登（詹姆斯·克伦威尔）。众多病患中，亦不乏宣称入院是遭到冤枉的，包含女同性恋记者拉娜·温特丝（莎拉·保罗森）、遭指控为连续杀人凶手的基特·沃克（伊凡·彼得斯）、性成瘾症花痴雪莉（科洛·塞维尼）以及涉嫌杀害继父母的少女凶手葛蕾丝·伯特兰（丽兹·布罗歇蕾）。
1949年时，茱德曾是夜总会驻唱歌手，风骚满城的她总能吸引男客目光，某日，下班後因酒驾撞死了一名小女孩，当时因过度惊慌未下车察看伤势便迳行离去，之後，茱德满怀歉疚与恐惧地过著生活。这也是她放下光环，转道投身宗教的原因，并选择了「荆棘崖」服务。修女玛莉·尤妮丝个性单纯温柔，对於修女茱德相当敬畏。之後，在为另一个病人进行驱魔过程时，纯净的玛莉成了魔鬼最好的寄宿场所，被附身的玛莉自此性格大变，成了残忍又任性的女人。她以制造各种幻象来嘲弄、威吓修女茱德酒驾肇事逃逸。惊恐失措的茱德，找上了女孩的家人，却才发现，当初女孩并未死亡，所幸只断了几根骨头，如今已然痊愈。此时，修女茱德意识到了这或许是上帝对她的考验，於是她决定消弭「荆棘崖」中所有剩馀的罪恶之事。
「罪恶之事」包括虐待狂科学家．亚登医师和病态连环杀手．崔德森医师。亚瑟·亚登来自前纳粹集中营，亚瑟於当时的集中营内以生人做活体实验，不少突变的病人成了「半兽人」，而如今这些半兽人被豢养在「荆棘崖」後方的树林中，不幸死亡（或遭亚瑟所杀）的患者屍体将被运来成为「盤中飧」。亚瑟与修女茱德在对於「荆棘崖」的经营方面与权利分配多有争执与冲突，水火不容。奥利佛·崔德森医生为由当局指派前往对於连环杀手．沃克进行评估与治疗的心理医生，然奥利佛实际上正为指控沃克为连环杀手的幕後指使人，而如此计画便是为了能让自己成为沃克的专属医生，进而谋杀他。沃克坚称他的妻子艾尔玛（布里妮·欧德福特）遭到外星生物所绑架，然而，奥利佛却设法将其他失踪及谋杀案加诸於其身上。沃克受到奥利佛道貌岸然外表下的诚恳词汇说动（实为诱导沃克说出假口供），使他涉嫌诸起谋杀案。而沃克也趁隙逃离看管所。
奥利佛也曾试著「改造」另一名因错误而被抓入「荆棘崖」的患者。拉娜·温特丝是一名雄心勃勃的报社记者，她试图揭开「荆棘崖」内惊悚骇人的内幕。她的小学老师女友．温蒂（可丽·杜娃），遭到修女茱德要胁签下拉娜的「卖身契」，否则将把她们的同性关系公诸於世，温蒂因而妥协（背叛拉娜的开端）。拉娜试图要求奥利佛帮忙联络温蒂时，奥利佛向拉娜坦承他就是凶手，而温蒂也已死於其手下。起初，奥利佛曾告诉过拉娜，凶手还仍在逃亡中。奥利佛暗中帮助拉娜逃出「荆棘崖」，却让拉娜得知了「血脸杀手」的秘密，而奥利佛则将多年来的思母之情（从未有过）投射於拉娜身上。之後，奥利佛强行和拉娜发生了关系，也险些遭到他的杀害。拉娜几经折腾後，逃离了奥利佛的住所，只得仓皇地回到疗养院。日後，拉娜发现她怀上了奥利佛的骨肉。
